The Five Patient Form Steps

Overview

When a patient opens their Patient Portal link, the intake forms are broken into five short sections they complete one at a time. Each section stands on its own — a patient can finish some now and come back for the rest — and everything they submit flows straight into their record in TeamCare.

This article walks through all five sections in order, what the patient provides in each, and exactly what counts as "complete." Use it to answer patient questions, to understand the "X / 5 complete" progress you see in Huddle, and to know what's still missing when a section shows as unfinished.

Each section carries its own status — Not startedIn progress, or Completed — calculated automatically from what the patient has entered. Nobody on your team marks a section done; it updates on its own as the patient fills in fields.

Step 1 — Personal Information

The patient reviews and completes their basic profile:

  • Name — first, middle, and last

  • Address — street (line 1 and line 2), city, state, and ZIP

  • Contact details — email and mobile phone

  • Emergency contacts — name, relationship, and phone for one or more contacts

Note: The portal only fills in email and mobile phone when those fields are currently blank on the patient's record. If you already have a phone or email on file, the patient's entry won't overwrite it — this protects your existing contact data.

Counts as complete when: the patient's core personal details are filled in.

Step 2 — Insurance Intake

The patient tells you how they'll be paying — through dental insurance or as self-pay.

If they have dental insurance, they provide:

  • Carrier name (their insurance company)

  • Plan type — PPO, HMO, DHMO, EPO, Indemnity, or Discount

  • Policy holder — themselves, a spouse/partner, a parent/guardian, or someone else

  • Member ID

  • Group number

  • Photos of the front and back of their insurance card

If they don't have dental insurance, they mark themselves as self-pay (not using dental insurance).

Counts as complete when: the patient has entered a carrier, member ID, and policy holder — or has answered that they're self-pay. Plan type, group number, and the card photos are helpful but not required for the section to count as complete.

Step 3 — Medical History

The patient shares anything the clinical team should know before treatment:

  • Allergies

  • Medical conditions

  • Medications

  • Notes — a free-text field for anything else

If the patient has nothing to report, they can check "no known medical conditions," which clears the lists and completes the section in one step.

Counts as complete when: the patient checks "no known conditions," or enters at least one allergy, condition, medication, or note.

Step 4 — Consent Form

The patient reviews and agrees to your practice's consents:

  • HIPAA consent

  • Communication consent

  • Signature — drawn right in the portal

Counts as complete when: both consents are agreed to and a signature has been drawn.

Step 5 — Preferences

Finally, the patient tells you how and when they'd like to be seen and contacted:

  • Appointment preferences — early morning, afternoon, evening, and/or weekend

  • Contact preferences — text message, email, and/or phone call

Counts as complete when: the patient submits their preferences.

How Progress Adds Up

As each section is completed, the patient's overall progress ticks up toward 5 of 5. Your team sees this same count in Huddle, on each appointment's Patient Intake Forms tile — for example "3 of 5 completed." See Reviewing Incoming Patient Forms in Huddle to find and read submitted forms.

Frequently Asked Questions

Q: Does a patient have to finish all five sections at once? No. Each section stands alone and saves on its own. A patient can complete some now and return later through their portal link to finish the rest. (Remember portal links expire after 30 minutes — if theirs has lapsed, send a fresh one. See Sharing a Patient's Portal Link.)

Q: A patient has insurance but the section still shows incomplete. Why? The insurance section needs a carrier, member ID, and policy holder to count as complete. If any of those is missing, it stays "In progress" even if the card photos were uploaded. Open the section to see what's still needed.

Q: What if a patient is paying out of pocket? They mark themselves as self-pay ("not using dental insurance") in the Insurance Intake step. That's a complete answer — the section will show as done.

Q: Does "no known medical conditions" really complete the medical section? Yes. Checking that box is a complete answer and clears any previously entered allergies, conditions, and medications.

Q: Can a patient update their answers later? Yes. The portal always shows their current information, so they can open it again to review and update any section.
