Reviewing Medical History & Insurance at Each Appointment

Overview
Intake forms are filled out once, but a patient's medical history and insurance can change between visits — a new medication, a lapsed plan, a different carrier. So when a patient confirms an upcoming appointment through the Patient Portal, TeamCare also prompts them to review their medical history and insurance for that visit, and keeps a record of what they confirmed for each appointment.
This gives your team a fresh, per-visit snapshot of two of the most important intake sections — without asking the patient to redo their whole intake every time.
Before you start: This review is part of the appointment-confirmation experience in the portal. The forms themselves are governed by Patient Forms Access — see Enabling the Patient Portal.
What the Patient Sees
When a patient opens their portal to confirm an appointment, they:
See the appointment details — date, time, provider, and type.
Are shown their medical history and insurance on file and asked to review them for this visit.
Update anything that's changed, or confirm that their information is still current.
Confirm the appointment ("Yes, I'll be there").
Because the review is tied to confirming the visit, patients handle it at the natural moment they're already thinking about the appointment — instead of it being a separate task that's easy to skip.
Why Medical History and Insurance Specifically
These two sections are the ones most likely to change between visits and most important to have right before the patient is in the chair:
Medical history affects clinical safety — allergies, conditions, and current medications.
Insurance affects eligibility and billing — a changed carrier, member ID, or a plan that's no longer active.
Confirming them at each appointment means your clinical and front-desk teams are working from current information on the day of the visit.
A Record Is Kept for Each Appointment
TeamCare keeps a record, per appointment, of the patient's medical-history and insurance review at confirmation. That means you're not just seeing a single "latest" answer — each visit carries its own confirmation, so you can tell what the patient reported for a given appointment.
Any updates the patient makes flow back into their record in TeamCare, so the changes are reflected going forward and are visible to your team.

Frequently Asked Questions
Q: Does the patient have to fill out their whole intake again at each appointment? No. Intake is completed once. At confirmation, the patient only reviews their medical history and insurance and updates anything that changed — the rest of their intake carries over.
Q: What if nothing has changed since the last visit? The patient simply confirms their information is still current and continues confirming the appointment. Their existing information stays as it is.

Q: Can we see what a patient confirmed for a specific past appointment? Yes — a record is kept per appointment, so each visit carries its own confirmation of medical history and insurance rather than only a single latest value.
