Patient Forms & Consents in Huddle

Overview

The other half of the huddle is making sure each patient is ready to be seen — that their intake forms and consents are done before they sit down. Huddle shows each patient's form progress right on the appointment card and gives you tools to move it along, including texting the patient a link to finish on their phone.

These features are available when Patient Forms is enabled for your practice. If it's turned off, the forms tile and Patient Forms tab don't appear.

The Patient Intake Forms Tile

On the appointment card, the Patient Intake Forms tile shows a quick "X / 5 complete" summary with a progress bar. Clicking it opens the Patient Forms tab of the opportunity window.

The Patient Forms Tab

The Patient Forms tab is where you manage the check-in interactions around intake forms and consents.

Form progress

A progress summary spans the five patient-portal form sections. The bar is amber while forms are in progress and turns green once all five are complete.

There's a card for each section, showing its status:

Each card shows one of three statuses:

Completed

The patient has finished this section.

In progress

The patient has started but not finished.

Not started

Nothing submitted yet.

Clicking a section card opens that patient's Profile → Patient Forms sub-tab in a new browser tab, so you can review or complete it on their behalf.

The patient portal link

Below the sections is a patient portal link card with controls to copyopen, or regenerate the patient's personal portal link — handy for reading the link to a patient over the phone or opening it yourself.

Texting the Portal Link

The "Text magic link to patient" button sends the patient's portal link straight to their mobile number so they can complete their intake forms and consents from their phone.

  • On success you'll see a confirmation ("Portal link texted to …"), and the button stays available so you can send it again.

  • If the patient has no mobile number on file, Huddle tells you instead of sending.

Frequently Asked Questions

Q: I don't see the forms tile or Patient Forms tab. Why? Patient Forms must be enabled for your practice. When it's off, both are hidden.

Q: Does opening a form section or texting the link change the patient's chart? No. These are check-in aids. Records change only when a form is actually completed — by the patient or by you on their behalf.

Q: The bar is amber even though several sections are done. Is that a problem? No. The bar stays amber while any section is still incomplete and turns green only once all five are complete.

Q: The text didn't send. What happened? The most common reason is that the patient has no mobile number on file. Add one to their record, then send again.

Q: Can I fill out a patient's forms for them? Yes. Click a section card to open that patient's Profile → Patient Forms sub-tab and complete it on their behalf.
